PPO vs HMO vs EPO

Three network designs, three different answers to the question of who you can see.

The difference is network behaviour

PPO, HMO and EPO are not quality grades. They describe how a plan treats providers inside and outside its network, and how care is coordinated.

A PPO generally allows both in-network and out-of-network care, with a larger share of the cost falling to you outside the network. An HMO usually concentrates care in-network and often uses a primary care physician to coordinate it. An EPO also tends to keep care in-network, typically without the referral requirement.

How to choose between them

Write down the providers and facilities you want to keep. Check each candidate plan's directory for those exact names. If they all participate in a narrower network, the flexibility of a broader design may not be worth the extra monthly cost. If they don't, that flexibility may be the whole point.
